Explanation of the Donation Form
Item Name: Title of your donated item
Estimated Value: Suggested price
Item Description: Just a quickie so we can identify it on the donation table. We will display your item in the store with a card saying who made it and the title only.
Would you like any unsold pieces returned after Arborfest?: Let us know if so. We will hold them aside for you and return them after the fest.
Can you deliver your own items: Please delivery all completed items to Arbor by the 1st Oct. We will have a table set up in the Arena two weeks prior to Arborfest.
I am a(n): Gotta check a box, just pick one.
I would like to remain anonymous: Again, gotta pick one. This is not a smart form.
Number of items to list: If you know, please list how many you are making here.

Due Dates:
- Form submission deadline: October 17th
- All handmade items need to be finished & delivered to the Arena by November 6th. We have a table set up here now and items can be dropped off anytime.
You have the option of making your items ahead of time or on-demand (i.e., make a single piece for listing & photo purposes, then plan to make only as many as are sold during Arborfest). Please be sure to list above how many you are willing to make so we don't oversell.
