Explanation of the Donation Form
Item Name: Title of your donated item
Estimated Value: Suggested price
Item Description: Just a quickie so we can identify it on the donation table. We will display your item in the store with a card saying who made it and the Item name/title only.
Can you deliver your own items: Please deliver all completed items to Arbor by Oct. 1st, the Friday before Arborfest. We will have a table set up Sept. 20th in Stables East for the two weeks prior to Arborfest.
I am a(n): Gotta check a box, just pick one.
I would like to remain anonymous: Again, gotta pick one. This is not a smart form.
Number of items to list: If you know, please list how many you are making here.
Would you like any unsold pieces returned after Arborfest?: Let us know if so. We will hold them aside for you and return them after the fest. Or, if you would like us to hold onto them for next year's fest, please let us know.

Due Dates:
- Form submission deadline: Thursday, September 30th
- All handmade items need to be finished & delivered to the Stables East classroom by October 1st. We have a table set up in the Stables East 2 weeks prior to the fest and items can be dropped off anytime (please label with your name!). IF you do need to drop something off Saturday (day of Arborfest), please email cheyenneweil@gmail.com to let us know (for example, very perishable items or if you are unable to drop it off sooner, etc.).
